Form Submission Instructions

Before you start, you may need to ensure you have all the data required to complete the application.
Previous addresses and postcodes, NI number, Passport Number, Driving Licence number.

Also a signed waiver, an ID document and proof of residence document. These can be submitted later or as part of the process. If you wish to upload documents you can use https://ftp.eurobg.com/

To complete the form answer all the sections that apply to your application and also to you personally.
The form will not submit unless all the asterisked fields have been completed.
The fields are in free format so you can input dates in your normal way.
Once you have completed all of the relevant parts then press the submit button at the top of the page.
You only need to do this once.

After the application has been submitted , you will see a page thanking you for your data.
There are then some documentation details. At this point you can download and sign a waiver and then upload any documentation.
